የይለፍ ሰነድ
- በአግባቡ በአመልካች የተሞላ እና የተፈረመ የማመልከቻ ፎርም (Laissez Passer form)
- ሁለት (2) የፓስፖርት መጠን ያለውና ቢያንስ ከ6ወር ወዲህ የተነሱት ጉርድ ፎቶግራፍ፣
- አገልግሎቱ ያበቃዉን ፓስፖርት መረጃ ያለበትን ማለትም ስም፣ ፎቶግራፍ፣ ፓስፖርቱ የተሰጠበትና ለመጨረሻ ጊዜ የታደሰበትን ገጽ የሚያሳይ ኮፒ ማቅረብ፣
- ፓስፖርቱ ለጠፋበት አመልካች የፓስፖርት ቅጂ እና የፖሊስ ሪፖርት፣

Laissez-passer
- Dully filled and signed application form (Laissez Passer form)
- Two (2) passport size photographs taken at least 6 months ago;
- Provide a copy of the completed passport information including the name and where the passport was issued and last updated;
- An applicant who has lost his/her passport shall submit a copy of the passport along with a police report;
- For applications submitted for a newly born latest passport size photographs (3 cm x 4 cm) colored with white background, clearly showing the front portion (head and face should be uncovered);
